Boston, MAGeneric medications currently account for over 70 percent of prescriptions dispensed. However, while generic drugs are clinically bioequivalent to the brand-name version, they often differ in their physical characteristics, such as color and shape. Researchers from Brigham and Women's Hospital (BWH) have found that some patients who receive generic drugs that vary in their color are over 50 percent more likely to stop taking the drug, leading to potentially important and potentially adverse clinical effects.

The study will be published electronically on December 31, 2012 in the Archives of Internal Medicine.

"Pill appearance has long been suspected to be linked to medication adherence, yet this is the first empirical analysis that we know of that directly links pills' physical characteristics to patients' adherence behavior," explained Aaron S. Kesselheim MD, JD, MPH, assistant professor of medicine in the Division of Pharmacoepidemiology and Pharmacoeconomics at BWH, and principal investigator of this study. "We found that changes in pill color significantly increase the odds that patients will stop taking their drugs as prescribed."

The researchers conducted a case-control study of patients taking antiepileptic drugs and compared the odds that patients who did not refill their medication had been given pills that differed in color or shape from the prior prescriptions. Using a large national database of filled prescriptions, when the researchers identified a break in the patient's use of the drug, they looked at the previous two prescription fillings to see if they were the same color and shape. They found that interruptions in the prescription filling occurred significantly more frequently when the pills had different color. Interruptions in antiepileptic drug use for even a few days can raise the risk of seizure and have important medical and social consequences for patients.

These findings offer important take-home messages for physicians, pharmacists, and patients. As Kesselheim explained, "Patients should be aware that their pills may change color and shape, but that even differently-appearing generic drugs are approved by the FDA as being bioequivalent to their brand-name counterparts and are safe to take. Physicians should be aware that changes in pill appearance might explain their patients' non-adherence. Finally, pharmacists should make a point to tell patients about the change in color and shape when they change generic suppliers."

Researchers acknowledge that medication adherence is a multi-faceted issue, but suggest that taking steps to permit (or even require) similarity in pill appearance among bioequivalent brand name and generic drugs may offer a relatively simple way to contribute to better adherence.

News Corp buys regional sports channel in Ohio

(Reuters) - News Corp said on Friday it bought SportsTime Ohio, a Cleveland-based regional sports network that broadcasts Cleveland Indians baseball games.

The company did not disclose financial details. A source close to the matter said on Friday the deal was worth roughly $230 million, which Reuters first reported on December 3.

The network, owned by the Cleveland Indians baseball team, also airs programming on the Cleveland Browns National Football League team, college football and high school sports.

Fox said it a statement it also struck a separate long-term TV rights agreement to air Cleveland Indians baseball games. Fox will pay the team about $40 million per year for more than 10 years as part of a new agreement, according to the source close to the matter.

Fox already owns a regional sports channel in Ohio, called Fox Sports Ohio, which is available in 5 million homes. It held the rights to Cleveland Indians games until 2006.

Fox Sports, which operates or holds stakes in 20 regional sports networks, provides sports programming to more than 67 million subscribers.

New York-based News Corp has been stepping up efforts to control the rights to key sports teams in response to Time Warner Cable Inc's $3 billion deal in February 2011 to carry the Los Angeles Lakers basketball games on its Time Warner SportsNet Channel.

Last month, Rupert Murdoch's News Corp said it would buy a 49 percent stake in the YES network, a sports channel controlled by the New York Yankees baseball team, a deal that sources said was valued at $3 billion. Reuters has reported that Fox is also negotiating a 25-year extension of its existing agreement to carry Los Angeles Dodgers baseball games and could pay as much as $6 billion for those rights.

News Corp is planning to separate its publishing and entertainment assets into two publicly traded companies next year. News Corp's regional sports networks, film and television businesses, which currently include the 20th Century Fox film studio, Fox broadcasting network and Fox News channel, will be part of the renamed parent company that will be called Fox Group.

News Corp shares were trading 9 cents lower, or 0.3 percent lower at $24.81 per share.

Sales Cycles ? Small Business Marketing

Many of my small business marketing peeps never take the time to really look at and consider sales cycles in their marketing efforts. Today?s post is going to be very important and worth taking the time to read!

Small Business Marketing - Sales Cycles and When Will They BuyThink of it this way, when you are shopping for anything, you have a process you go through that starts with the idea and ends with the purchase. If it is a tiny item like a candy bar, the thought and the purchasing may be very close. If it is a large item or something you need for your business or to help improve your life, the timeline may be considerably longer, with more research involved.

Thinking of buying ? So you are sitting at your desk and contemplating lunch when suddenly you see an ad for item, OR you are at an event and a speaker talks about something that is new to you and you think, ?hmmm that might work for my business?, OR you are talking with a friend and they tell you about this great new product or service they are using. All of these are ways that the IDEA of buying something can come into your head.

Early research ? Now you know this thing exists and if someone doesn?t sell you that minute on it, you are probably going to do some research on the topic.

I heard a great example somewhere about guinea pigs. People rarely buy guinea pigs spontaneously (thank goodness). They see them (or more likely their kids see them), then the parents research how much care and feeding they take, then the kids beg for a while and finally a pig is purchased.

There are multiple places in this research phase that you could attract the attention of researchers, have a GP for Dummies Book, offer a free guide to how to pick a long living GP, have a video tutorial about the care and cleaning of a cage. All of these things will get them into your sales pipeline IF you capture their data by asking for their email address and name.

Middle research ? Okay, the initial discovery had been done and now your customer is moving onto serious research. In this case lets look at a bride. She has become engaged, done some early dreaming about her wedding, but now the rubber hits the road and she needs to find out pricing and availability of venues, where to get her gown and all the other things a bride has to do.

As a vendor in the wedding space, you would want to be on this short list. What if, during early research you had offered something like a free wedding checklist that she could use to make sure everything gets done and have continued to send tips her way for the last couple of months? Don?t you think your company will be on that short list?

Buying ? The research is done and they are ready to buy. This is the sad little place that I see too many of my peeps miss the mark. They are all gung ho up in the research phase and actually convince someone of the value of buying whatever they sell, but then peter out and functionally make the sale for whoever comes along next and talks about it. You HAVE to be in constant contact at this point to make the sale!

The Bad News About Sales Cycles

I have bad news, most of my clients come to me all bright eyed and bushy tailed saying that they only want to target people who are 5 minutes away from buying their product or service. That is a truly admirable goal and sometimes possible by targeting buying keywords in your organic or paid online marketing, but mostly people want to buy from someone they know, like and trust.

Let?s say a customer is looking for social media marketing services and somewhere early in their search they happen along my site. They sign up for my mailing list and we send them great information for weeks or months about how to do their social media, what to look for, etc. By the time they are ready to buy they ?know? me. They have seen my videos, they have read my emails, I have helped them to understand what they need to do and what kind of company they need to look for (miraculously close to what our company does) and so their buying decision becomes no decision, OF COURSE they hire us because we are the only real choice.

How To Do Sales Cycle Marketing

So you are thinking, ?bloody hell?, how am I going to get all these people from thinking about buying to finally picking up the phone to call with a sale. You need to have a database that has the capability of:

  • Having a web form to collect the suspect?s email address and name
  • Have a series of ?drip emails? (also called auto responders) that keep you in the front of their thoughts
  • A way to start separating the wheat from the chaff into prospects by checking which links they click in the emails to see where they are in the sales cycle
  • The ability to delegate some of these tasks (I have a WONDERFUL executive assistant who makes most of my calls for me, you NEED to have the best person for the job getting the task to perform)
  • Continuous, gentle reminders along the way to buy from you (these do not have to scream buy now, buy now, but why miss a sale because someone doesn?t know you are selling)

Actress Katie Holmes' Broadway show to close

NEW YORK (Reuters) - Actress Katie Holmes' return to Broadway has been cut short, with producers announcing that the play "Dead Accounts" in which she co-stars will close on January 6, nearly two months early.

Holmes, the ex-wife of actor Tom Cruise, played Lorna, a wan, beaten-down woman living with her parents in the five-character play by Theresa Rebeck which opened on November 29 to mostly negative reviews.

No reason was given for the play's early closing, but media reports said it was earning only a fraction of its box office potential.

Many reviewers said Holmes acquitted herself alongside a roster of Broadway veterans, who included Tony-winning actor Norbert Leo Butz as the brother who returns to his Midwestern family and unleashes havoc in the comedy.

The New York Daily News said "she throws herself gamely into her second Broadway show ... (but) Holmes' efforts add up to zilch."

Most critics laid blame on an undeveloped, sketchy play by the author of last season's better-received "Seminar."

Holmes, 34, reached a high-profile divorce settlement with Cruise last summer. She lives in New York with her young daughter, Suri. Holmes will co-star in an upcoming film which will be a modernization of Chekhov's "The Seagull" along with Allison Janney and William Hurt.

Gaming prowess rarely comes cheap, but that doesn't mean it can't at least be affordable. The Asus G75VW-DH72 gaming laptop may offer pricing and performance well below what tricked out systems like the Eurocom Scorpius can offer, but when stacked against mid-range gaming systems like the Alienware M17x R4, it still holds its own, getting you on the gaming grid for under two grand.

Design and Features
While previous incarnations of Asus' Republic of Gamers line have emulated stealth fighter jets and naval vessels, the G75VW's design is a little more conservative, especially for a gaming machine. It's not as slim as the Razer Blade (2012), by any means. The laptop is still large, measuring 2.0 by 16.3 by 12.6 inches (HWD), but has changed up the look a bit, adding metal accents around the keyboard and across the speaker grille, which break up the imposing all-matte-black look seen in designs past. The result is a gaming rig that looks almost professional. Now, don't misunderstand, at 8.7 pounds (10.9 with the power brick) you won't be lugging this laptop to and from work, but it wouldn't look completely out of place in an office environment.

Despite the conservative look, the gaming DNA is still there. The back end of the G75VW is dominated by large cooling vents, and Asus' designs are tweaked to keep the system cool but quiet, grouping the heat generating components toward the back of the system. The chiclet keyboard sits at a slight slope, but is otherwise competent, providing a shallow but firm key movement. The touchpad is the same high-quality touchpad found on the Asus G75VW-DS71, but with the shift to Windows 8, Asus has also added some gesture support for Windows 8-specific controls. A swipe from the right edge of the touchpad pulls up the Charms Bar, while a three-fingered swipe down cycles through open apps. Pinch and zoom and two-finger scrolling aren't new additions, but they take on new importance in the absence of a touchscreen.

You may not be touching the screen, but the 17.3-inch display is still bright and sharp, with 1920-by-1080 resolution providing 1080p whether you're playing Skyrim or watching The Dark Knight on Blu-ray. The sound is also pretty good (as tested with the new trailer for The Man of Steel), staying clear at high volumes and providing a fair amount of bass thanks to an integrated subwoofer.

In addition to a Blu-ray/DVD combo drive, the Asus G75VW is outfitted with four USB 3.0 ports (one with power for charging devices), Gigabit Ethernet, a 3-in-1 card reader (SDHC, MMC, MS), and jacks for headphone and microphone. You also get several options for video connectivity, with HDMI, VGA, and mini DisplayPort outputs. Wireless connections provide 802.11n Wi-Fi and Bluetooth 4.0.

The G75VW-DH72 pairs a 750GB spinning hard drive with a separate 256GB solid-state drive (SSD), letting you shift bulk data storage to the hard drive while keeping programs on the SSD for optimum performance. Included on the drive are Microsoft Office Starter 2010, a 60-day trial of Trend Micro Antivirus, Adobe Reader X, and Asus' Application Suite, which includes 3GB of free cloud backup with Asus WebStorage. Asus covers the G75VW-DH72 with a two-year warranty, but also includes a full-year of additional accidental damage coverage, 24/7 tech support, and 30-day "Zero Bright Dot" warranty on the display panel.

Performance
Asus G75VW-DH72 The Asus G75VW-DH72 is outfitted with a 2.4GHz quad-core Intel Core i7-3630QM processor with 16GB of RAM, and an Nvidia GeForce GTX 670M with 3GB of dedicated VRAM. The combination is potent?in Cinebench R11.5, the processor scored 6.44 points, outperforming both the Alienware M17x R4 (6.19 points) and the MSI GT70 0NE-276US (6.23 points), but falling behind both the Razer Blade (6.86) and the Eurocom Scorpius (7.29). The Asus G75VW-DH72 also made short work of our multimedia tests, completing our Handbrake video encoding test in 36 seconds, and Photoshop CS6 in 3 minutes 32 seconds.

Perhaps more important than general performance is how the G75VW-DH72 does with games. In Aliens vs. Predator, it racked up a respectable 55 frames per second (fps) at medium detail settings and 1,366-by-768 resolution, but dropped to a less than playable 18 fps when resolution was dialed up to 1,920-by-1,080. When tested with Heaven, however, it scored 70 fps at medium settings and a nearly playable 28 fps at 1080p. While you won't be able to go all out with the detail settings on high-end games, you will still see excellent performance on even the most demanding games, provided you back off on the eye-candy a bit.

The Asus G75VW-DH72 also did well in battery tests, lasting 3 hours and two minutes in our battery rundown test. While not even long enough to make it through a screening of Kurosawa's Seven Samurai, it's still pretty decent performance for a gaming laptop. Few laptops pass the three hour mark, with one of the few exceptions being the portability minded Razer Blade, which still only lasted 3 hours 53 minutes when tested with MobileMark 2006, our previous battery test.

As a decently priced mid-range gaming laptop, the Asus G75VW-DH72 offers great performance at an affordable price, letting you enjoy top of the line games (albeit with middle of the road graphics performance) for far less than a tricked out high-end system. It's an affordable option to the Alienware M17X R4, and as such, replaces it for our Editors' Choice mid-range gaming laptop.

Orlando mom adopts 4 from foster care, quickly loses 2 boys to rare genetic disease

MIAMI ? When Felicia Peete adopted four young siblings from foster care, she moved out of her Orlando area retirement community into a big new home where they could sing and roller-skate, far away from the memories of living in cars and homeless shelters. She imagined birthday and graduation celebrations with her four great nephews and niece, never dreaming that within two years she?d be planning funerals for her eight and nine-year old sons.

The children, all under the age of seven, had a tumultuous two years before 55-year-old Peete adopted them in 2010. They had stopped going to school, their father went to prison and their mother was struggling with drug abuse. They lacked basic hygiene and didn?t know how to use toilet paper or brush their teeth. The training was arduous at times with four little ones, but ?Mama Gail? as they called her, was good natured and she longed to make up for their past hurts.

But several months after the adoption, Peete worried something was wrong with then 7-year-old Tyler. The math whiz with a sophisticated vocabulary went from helping his siblings with their homework to getting D?s and F?s in school. Doctors treated him for hyperactivity disorder, but the symptoms progressed. Tyler began drooling and dragging his right leg.

After a series of lengthy genetic tests, Tyler was diagnosed with Adrenoleukodystrophy (ALD), a rare genetic disease that typically strikes young boys, leading to blindness, deafness and dementia and killing them within a few years.

And then the unthinkable happened. Tyjuan, then seven, and five-year-old Tyveon were diagnosed several months later. Their sister Tyanna was spared.

?I was just really scared. I wanted to just grab them and save them and take them away. I wanted to be God,? said Peete.

Tyler?s symptoms progressed quickly. Within four months he?d stopped walking, lost his sight and needed a feeding tube.

When Tyler struggled to walk, Tyjuan grabbed his hand and helped him along. When Tyler had a bathroom accident, Tyjuan bathed him.

Peete was honest with the children about Tyler?s condition.

Tyjuan rubbed Tyler?s forehead and straightened his blanket the day before he died in hospice care last January.

?You can?t get used to that, you can?t prepare yourself to that,? said Peete, her voice breaking.

Then, little Tyveon was hospitalized for nearly four months after his brain started swelling. In the hospital bed next to his, his brother Tyjuan, was recovering from a stem cell treatment Peete hoped would save his life. The boys walked the hospital hallways together, dragging along their metal polls with medical tubes.

Peete spent two months in a North Carolina hospital where Tyjuan underwent three unsuccessful stem cell transplants.

No longer able to talk, the always affectionate Tyjuan often signed ?I love you? and, blew kisses to comfort ?Mama Gail?. Peete helped him Skype with his two siblings back in Orlando and tried to stay positive.

?About a week before he died he looked at me and smiled and the tears rolled down his eyes like he knew what was going to happen and he knew I was going to be in pain. He just looked at me and I knew he was saying, ?I love you, Mama, I?m going to be OK,?.?

The eight-year-old died in the hospital this past fall, away from his beloved siblings in a strange city. Peete waited to tell the other children in person that Tyjuan was in heaven with Tyler helping God watch over them.

As the family reeled from the boys? deaths, medical expenses were mounting. Tyveon was on a dozen medications a day. Peete barely had time to pay off Tyler?s funeral and now she was planning Tyjuan?s. She lost her sales job at a cable company and took a pay cut to work from home to as the boys? medical needs grew. This December, she moved the family into a smaller house along with her two grown daughters, who often watch the children.

Peete tried to comfort herself by watching videos of the boys singing and dancing, but she grew angry at God and stopped going to church. She blamed child welfare workers for not diagnosing the boys while in foster care and blamed herself for not being able to save them.

Tyanna has grown quiet and angry. Tyveon often plays a gospel song about heaven that reminds him of his brothers and asks to go to church.

But the community has rallied around them.

An anonymous donor stepped forward this month to pay off $1,700 of Tyjuan?s outstanding funeral bill. The Department of Children and Families worked with organizations to buy them nearly $600 worth of Christmas presents.

?Through all of the heartache and loss of both boys, loss of her job, having to move into a smaller home, and getting treatment for Tyveon, Mrs. Peete has been kind and hopeful,? said Salena Burden, an adoption caseworker.

Tyveon?s treatments have been successful because he was diagnosed at a younger age than his brothers. As Peete watches, she said her raw emotions are healing.

Last Sunday, she went back to church for the first time since the boys? deaths.

What Debris From North Korea's Rocket Launch Shows

I was wondering whether the analysis was just based on video frames (since they talked about the colors of the flames and such) in the "AllThingsNuclear.org" article. The
article [allthingsnuclear.org] itself says that the analysis is based upon four pieces of the first stage of the Unha-3 rocket recovered by South Korea. The author of the article, David Wright [allthingsnuclear.org], surmises that all four pieces came from the first stage because they "were found in the same area".

The four parts found were:
1 -- oxidizer tank (made of an aluminum-magnesium alloy)
with a cool picture (fig 4) of the inside of the tank showing hoops and stringers supporting the wall
2 -- two bottles that make the "turbo pumps" to maintain pressure in the oxidizer tank as the fuel flow continues during launch
3 -- another part of the fuel tank (with the number "3" painted on the outside which is visible on the launch video)
4 -- what appears to be a support ring from the first stage body

There's also a comment at the end about using "room temperature fuels" such as RFNA (red fuming nitric acid) allowing the use of a simplified design as compared to using cryogenic fuels which require a more complex design. Someone wrote in pointing out that RFNA is also used in the Russian Kosmos 3M [wikipedia.org] space launch vehicle which is also derived from a ballistic missile. In fact, even the fins and the profile of the Kosmos looks like the fins on and the profile of the North Korean launch rocket. Pretty cool analysis, and I like that the author puts really links to the sources of the pictures he has in the article.

I knew that it would be something drastically different from the course that I had embarked on in 1988 as a bright-eyed, newly graduated genetics student. My growing concern for the economic advancement of women, workforce development, and a desire to be my own boss urged me to move in a new and different direction.

While working in a corporate setting for 22 years, I went back to school and obtained a master?s degree in human resource development. For years, I did pro bono work to gain experience. I launched my first consulting practice in 2004.

Today, I provide human resource development consulting to individuals and nonprofit organizations. I am having the time of my life, and this is truly what many people refer to as an ?encore career.?

What is an ?encore career??

After career transitions, retirement, life changes, etc., you may find yourself having to regroup and start over again. Depending on your age and financial status, you may not be able to retire in the traditional sense and collect benefits or a pension. Many people find that they just aren?t ready to stop contributing as part of the American workforce.

A growing trend is to use your authentic vocation or passion to meet a need and get paid for it. These are also called ?encore careers.? We often think of retirees as having the time and freedom to do what they love, but younger boomers (45-55) are finding encore careers to be very fulfilling and lucrative.

According to Wikipedia, an encore career is work in the second half of life that combines continued income, greater personal meaning, and social impact. These jobs are paid positions, often in public interest fields such as education, the environment, health, the government sector, social services, and other nonprofits.

Getting started

Before you get started, make a list of skills and abilities that you possess. In particular, list items that you can turn into a business. Editing, dog walking, r?sum? writing, hairstyling and writing are a few businesses that can be started as home businesses. But remember, your clients will expect the same quality that your employer expected of you, with the addition of this special ingredient: great customer service! Customer service can make a business thrive or cause its demise.

If you choose to use your corporate experience to do good, let everyone know. Join associations and let key decision makers and influential people know that you are on the market and looking for a way to leave your legacy and leverage your corporate experience in a way that is satisfying to you.

Barry Bluestone, dean of the School of Public Policy and Urban Affairs at Northeastern University, predicts that within the next eight years there could be at least five million potential job vacancies in the United States, nearly half of them (2.4 million) in social sector jobs in education, health care, government and nonprofit organizations. The loss in total output could limit the growth of needed services and cost the economy as much as $3 trillion over the five-year period beginning in 2018.

Bluestone?s analysis builds on the ?2008 MetLife Foundation/Civic Ventures Encore Career Survey? conducted by Peter D. Hart and Associates. This work shows that most people expect to work longer than previous generations, but that half of those aged 44 to 70 want encore careers. ?Not only will there be jobs for these experienced workers to fill,? Bluestone writes, ?but the nation will absolutely need older workers to step up and take them.?

Genetic sequencing breakthrough to aid treatment for congenital hyperinsulinism

Dec. 27, 2012 ? Congenital hyperinsulinism is a genetic condition where a baby's pancreas secretes too much insulin. It affects approximately one in 50,000 live births and in severe cases requires the surgical removal of all or part of the pancreas.

Researchers at the University of Exeter Medical School are the first in the world to utilise new genetic sequencing technology to sequence the entirety of a gene in order to identify mutations that cause hyperinsulinism. Previously, existing technology limited such sequencing to only part of the coding regions of the gene which meant that some mutations were missed.

Using new Illumina genetic sequencing technology, the research team led by Professor Sian Ellard has discovered novel mutations that cause hyperinsulinism. Their findings are published December 27 2012, on-line by The American Journal of Human Genetics.

The outcome will be that some infants born with hyperinsulinism will require fewer investigations, because the new technology means that for many only one genetic test will be required to determine the extent of the condition in each child. It also means that clinicians will have more information at their fingertips to inform them about how much of the pancreas needs to be removed.

Approximately 50 per cent of patients with congenital hyperinsulinism require surgery, and of those half require the entire pancreas to be removed. Removal of the entire pancreas increases the risk of diabetes later in life, but if left undiagnosed and untreated hyperinsulinism can result in irreparable brain damage. Symptoms range from shakiness and tiredness to seizure and coma.

Dr. Sarah Flanagan, Research Fellow in Molecular Genetics at the University of Exeter Medical School said: "The potential provided by this new technology is important and exciting, because it allows us to investigate genetic coding in its entirety. This means that investigators can identify mutations that sit at the heart of any number of conditions where before they might have been missed. This in turn results in better information for clinicians upon which they can base effective treatments and interventions for their patients."

Newspaper's handgun-permit map draws criticism

NEW YORK (AP) ? A newspaper's publication of the names and addresses of handgun permit holders in two New York counties has sparked online discussions ? and a healthy dose of outrage.

The Journal News, a Gannett Co. newspaper covering three counties in the Hudson Valley north of New York City and operating the website lohud.com, posted a story Sunday detailing a public-records request it filed to obtain the information.

The 1,800-word story headlined, "The gun owner next door: What you don't know about the weapons in your neighborhood," said the information was sought after the Dec. 14 school shooting in Newtown, Conn., about 50 miles northeast of the paper's headquarters in White Plains. A gunman killed his mother, drove to an elementary school and massacred 20 first-graders and six adults, then shot himself. All the weapons used were legally owned by his mother.

The Journal News story includes comments from both sides of the gun-rights debate and presents the data as answering concerns of those who would like to know whether there are guns in their neighborhood. It reports that about 44,000 people in Westchester, Putnam and Rockland counties are licensed to own a handgun, and that rifles and shotguns can be purchased without a permit.

It was accompanied online by maps of the results for Westchester and Rockland counties; similar details had not yet been provided by Putnam County. A reader clicking on the maps can see the name and address of each pistol or revolver permit holder. Accompanying text states that inclusion does not necessarily mean that an individual owns a weapon, just who obtained a license.

By Wednesday afternoon, the maps had shared about 30,000 times on Facebook and other social media.

Most online comments have criticized the publication of the data, and many suggest it puts the permit holders in danger because criminals have a guide to places they can steal guns. Others maintain it tells criminals who does not have a gun and may be easier to victimize, or where to find law enforcement figures against whom they might hold a grudge.

Some responded by publicizing the home addresses and phone numbers of the reporter who wrote the piece, along with other journalists at the paper and even senior executives of Gannett. Many echoed the idea that publicizing gun permit holders' names is tantamount to accusing them of doing something wrong, comparing the move to publishing lists of registered sex offenders.

The Journal News is standing behind the project but did not immediately return phone calls seeking comment. It said in the story that it published a similar list in 2006.

"New York residents have the right to own guns with a permit and they also have a right to access public information," Janet Hasson, president and publisher of The Journal News Media Group, said in a story the paper published about the response to the database.

Roy Clark, a senior scholar at the Poynter Institute, a Florida-based journalism think tank, said publishing the data was "too indiscriminate."

He, too, compared the maps to similar efforts involving sex-offender registries or lists of those arrested for driving under the influence, noting that such a move is usually done to indicate a serious problem that requires a neighbor or parent to maintain vigilance.

"You get the connotation that somehow there's something essentially wrong with this behavior," he said of the gun permit database.

"My predisposition is to support the journalism," Clark said. "I want to be persuaded that this story or this practice has some higher social purpose, but I can't find it."

Also common among the comments on the lohud.com were suggestions about suing the paper for violating permit-holders' privacy rights. Such a move would likely be unsuccessful.

"The media has no liability for publishing public information," said Edward Rudofsky, a First Amendment attorney at Zane and Rudofsky in New York. The issue does present a clash between First and Second amendment rights, he said, but in general, the law protects publishing public information unless the intent was to harm someone.

UN envoy to Syria to visit Russia this weekend

MOSCOW (AP) ? The international envoy on Syria will visit Moscow this weekend for talks on the crisis, a senior Russian diplomat said Wednesday.

Deputy Foreign Minister Mikhail Bogdanov said that U.N.-Arab League envoy Lakhdar Brahimi will arrive in Moscow on one-day visit Saturday, according to the ITAR-Tass news agency.

Brahimi met Monday in Damascus with Syrian President Bashar Assad, but didn't signal any progress toward a negotiated solution for the civil war. Brahimi told reporters after the talks that he and Assad exchanged views on the crisis and discussed possible steps forward, which he did not disclose.

Preceding Brahimi's trip, Syrian Deputy Foreign Minister Faisal Mekdad and Assistant Foreign Minister Ahmad Arnous flew early Wednesday to Moscow, according to airport officials in Beirut. The Russian Foreign Ministry made no immediate comment about their mission.

Russia has used its veto right alongside China at the U.N. Security Council to protect its old ally from international sanctions over a civil war that has killed more than 40,000, but it has increasingly sought to distance itself from Assad.

Foreign Minister Sergey Lavrov said last week that Moscow would welcome any country's offer of a safe haven to Assad, but has no intention of giving him shelter if he steps down.

At the same time, Moscow has given no indication that it could end its firm opposition to international sanctions against Assad and calls for him to step down.

Lavrov reaffirmed Russia's call for the fulfillment of a peace plan brokered by Brahimi's predecessor, Kofi Annan, that was approved at an international conference in Geneva. On Russia's insistence, the plan left the door open for Assad being part of the transition process and has been rejected by the opposition.

Gene variants affect pain susceptibility in children

Dec. 24, 2012 ? At least two common gene variants are linked to "clinically meaningful" differences in pain scores in children after major surgery, reports a study in the January issue of Anesthesia & Analgesia, official journal of the International Anesthesia Research Society (IARS).

"[O]ur study is highly suggestive of a genetic component in pain response among children," concludes the study by Dr Chantal Mamie and colleagues of Geneva University Hospitals, Geneva, Switzerland. But an accompanying editorial question the relevance of this and previous studies of pain-related genes for management of pain in individual patients.

Gene Variants Influence Pain Scores after Surgery ?

The study was designed to explore whether several "candidate" gene variants affected pain scores in a group of 168 children undergoing major surgery -- either abdominal or bone and joint operations. The children and their parents were tested for variant forms ("polymorphisms") of six different genes previously reported as having a possible impact on pain.

The genetic results were compared with the children's pain scores, as routinely monitored during the 24-hour recovery period after surgery. During that time, the children had access to patient- (or parent- or nurse-controlled) analgesia with strong opioid (morphine-related) pain relievers.

Variants of two genes were related to "clinically meaningful" increases in pain scores -- at least four "peak" scores higher than six (on a ten-point scale) during the 24 hours after surgery. After adjustment for other factors, the risk of elevated pain scores was 4.5 times higher for children with a specific variant of the gene ABCB1, which affects the transport of opioid drugs to the central nervous system.

Risk of elevated pain scores was 3.5 times higher for children with a certain variant of the gene OPRM, a key target receptor for opioid binding. The associations with ABCB1 and OPRM variants remained significant after adjustment for patterns of gene inheritance from parents. Variants of two additional genes affecting pain perception -- NTRK and COMT -- were linked to more subtle, "subclinical" effects on pain scores.

? But Have No Effect on Use of Pain Medications

Surprisingly -- even though the gene variants affected pain scores -- they were unrelated to the total dosage of opioid medications used. The dosage of patient-controlled analgesia provides an important objective measure of pain and pain control after surgery.

"The present results are plausible given the known functionality of the candidate genes, and are consistent with the findings in adults," Dr Mamie and colleagues write. Although there has been a wealth of research on the genetic basis of pain in adults, the researchers add, "This first but small cohort study provides clues to further explore the genetic foundations of pediatric pain."

In an accompanying editorial, Drs Debra Schwinn and Ruth Landau of University of Washington, Seattle, put the findings in perspective. A decade ago, researchers thought that the discovery of genes affecting pain perception and opioid responses would soon play an important role in "individualizing" pain control after surgery. Subsequent studies have shown that the situation is more complex, and that the inheritance of pain susceptibility and opioid responsiveness is "probably less straightforward and predictable than previously foreseen."

Especially with the lack of effect on pain medication dosage, Drs Schwinn and Landau suggest that the presence of gene variants (genotype) may be less important than the way those genes are expressed in the individual (phenotype). Because of the complexity of the associations, they conclude, "[T]ailoring opioid analgesia based on selective genotyping is unlikely to occur anytime soon."

Top Diesel Myths and Why Diesel Hybrids Make Perfect Sense ...

Since 2004 I have been driving a diesel engine. The B5.5 generation VW Passat gets about 40 mpg despite being a full-sized vehicle with the towing strength equivalent of a Ford F150 pickup. It's really quite amazing to consider how efficient it runs. On a trip from San Francisco to Las Vegas, which is approximately 600 miles, I did not have to stop to refuel and saved almost an hour off the total time.

Even a small time spent looking at or driving a modern diesel vehicle tends to teach you several things about diesel myths:

  1. They're expensive: Despite being loaded with all the possible upgrades diesel cars can be less expensive off the line than a gasoline equivalent model. I paid $5K LESS in 2004 to get a diesel Passat instead of gasoline. When I told others they went to the dealer and confirmed the same thing. My parents bought a brand new 2005 VW Passat TDI after they saw the sticker price was lower than the sticker on gasoline. Diesel vehicles apparently can be priced very competitively. And then, after paying less, the value of my car actually increased.

    Some suggest diesel engines now must be more durable (due to compression load and high-pressure) and therefore more expensive to build than a gasoline engine. This is not, however, an argument against diesel. It could be another reason FOR buying diesel; a lot like saying a larger more powerful gasoline engine will be more expensive to build than a weaker small and inefficient one. Charging a premium for better performance/efficiency/durability obviously is no real barrier for auto sales, right Ford Tough?

  2. They're noisy: Disesel vehicles are quiet, really quiet. I've said this before but when the Audi and Peugeot hugely powerful (1,000 ft/lb torque) race cars ran the 24 hours of Le Mans the fans told me they were disappointed. You no no longer could hear the roar of the engines. The cars are incredibly powerful yet understated. It was the vehicles that lost after stopping more often in the pit to be refueled, such as Ferraris, Porsches and Corvettes, that made the most noise.

    Consider also that my mother, a professor who studied sound, in 2005 said she did not like the idea of buying a diesel because of the reputation for noise. She was bracing for what she had been told would be the stereotypical "clatter". However, when she sat down in the actual car for a test drive she said with delight "It purrs like a cat!" Seven years later she bought a second diesel and she says it is practically silent. This is no coincidence. Diesel engines run at lower revolutions to achieve power and therefore are quieter.

  3. They're anemic: Torque is an amazing thing for driving. Horsepower is what most people are used to in America. The horsepower feels great when you want to get going because you can rev an engine through a long curve. Step on the pedal and you might not take off right away but you can accelerate through 4000 rpm. It turns out torque is really what matters most. Driving up a hill at 65 mph and want to avoid shifting down? You need torque. Driving a car full of family and groceries yet trying to pass another car and want to avoid shifting down? You need torque. Torque is not only great for pulling weight, it also is great for getting started at slow or low speeds such as driving in slippery conditions.

    Diesels have a lot of torque but they also tend to have a short curve of power. Starting from a stop they can kick but then feel hesitant/gutless compared to a gasoline engine. This is typically resolved by the addition of a turbo to push through the upper end of a curve. Some even have twin turbos and/or technology to eliminate the lag when engaging a turbo. If you have ever been pushing a diesel pedal when the turbo fails you will be faced with the undeniable realization that it is a completely different beast than gasoline; some extra technology is needed to give it the exhiliration of horsepower.

    Another way of looking at this is the average mileage of diesel is impacted by stop-and-go. 40 mpg is easy on the highway because there's almost no need to touch the pedal but if you stop for a lot of red lights you have no choice but to push fuel. Another point on torque, My VW Passat wagon is a 4 cylinder engine. The power it produces at low RPM is comparable to a 6 cylinder gasoline engine. That's why diesel cars can be made with smaller, lighter engines yet have reasonable power.

  4. There's no room for improvement: The improvement over just a few years has been amazing. More efficient injectors, cleaner emissions, quieter, smoother?the list goes on and on. Innovation has allowed the latest VW diesels to achieve 90 mpg in track tests and 84 mpg in real-world use. Honda says its new Civic diesel engine runs 79 mpg with 221 ft-lb torque and is the lightest in its class. Subaru calls its 2005 boxer diesel engine a "True Engineering Revolution"

    When Subaru started its development project for the BOXER DIESEL, we soon realized that we were in an unprecedented, unchartered area in diesel engine development and were undertaking a technological challenge for which no benchmarking comparisons existed.

    You also could simply follow Catepillar's diesel innovation marketing from 2001 (clean bus fleets) to 2005 (low emission trucks) to 2012 (near zero emissions). Outside the consumer automobile, diesel is marketed with many amazing innovations. I've been eyeing the Volvo diesel hybrid (already sold-out for 2013) because it gets 126 or better mpg yet has the torque and all wheel drive of a truck combined with the performance of a Ferrari 308.

Perhaps you can see why I am so eager to stay with diesel when I move to a hybrid vehicle. Given that we're only just starting to see the potential innovation in both diesel and electric technology we could be on the verge of a vehicle revolution. Imagine combining the performance of a sports car, power of a utility truck and the efficiency of a daily driver into a single vehicle. That is what we see already in the first diesel electric hybrids.

Obviously an electric engine eliminates the stop/go mileage issue completely. Diesel only would be needed at speed and long distances, where it is getting more efficient every year. In other words:

Captain Obvious

It might sound obvious but I have to stop now and reflect on a strangely opposite view from a site called Green Car Reports. John Voelcker wrote an opinion piece called "Diesel Hybrids: Why They Don't Make As Much Sense As You Think". His arguments against using diesel hybrids are the following:

  1. "First and foremost is the issue of cost. On average, a diesel engine costs about 15 percent more to manufacture than a gasoline engine of equal output."
  2. "?a diesel hybrid should have boatloads of torque off the line, but may require extensive gearing to ensure highly efficient running at speed."
  3. "Gasoline engines convert 25 to 30 percent of a fuel's energy content into forward motion at the wheels; the rest is wasted as heat and noise. By contrast, a diesel converts 30 to 35 percent of the fuel's energy into forward motion?hence the higher fuel efficiency figures. But that leaves less "headroom" for improvement."

Ugh. No, really. Ugh. I want to put John in a diesel just for a day so he can feel how utterly wrong he is on point number two.

A horsepower curve continues well beyond the diesel curve as explained by The Institute of Motor Industry. The diesel has to shift or hit a turbo to get through the full acceleration path while a gasoline engine just revs higher and higher. What this really means is that he is flat wrong; diesel is a better match for hybrid because electric can carry the start to speed and then leave diesel to maintain speed with efficiency (as it does already). This is a PERFECT application of the electric engine that has NO CURVE. Let it take over starts/stops and you have a beautiful marriage of technology. John would instinctively know this if he drove a diesel.

Now back to point number one and the matter of cost.

I call bullshit. Cost is 15% more for an engine of equal output? Let's see the numbers on that unbelievable statement. Are we measuring patent application fees or what? Rather than get tied down in an inventory of parts and labor, however, let's get straight to the point. Nobody thinks the Ford Raptor is an inexpensive vehicle. A stock sports truck ready to chew up Baja desert at 60 mph will set you back $45K. And probably not one single Raptor sold will ever actually be used for what it was designed. So cost can be higher and people gladly pay more for it because of percieved and realized value, period. Hey, guess how much more it costs for those 22" chrome spinners?

Even if I go along with the unbelievable point that the diesel engine costs 15% more for "equal output", in terms of value it kills a gasoline engine with a longer life and higher efficiency. Go tell a family of four that they will have to visit the pump half as often and they will be glad to pay a premium. Just the other day a couple with a newborn child pulled up and told me they switched from a Tahoe to a Jetta TDI when they realized they would go from refueling every week to once a month. What is the value of all that time gained to parents of a newborn? John's 15% cost worry simply evaporates in the face of some common sense.

Now on to his third point.

He says there's no "headroom" for improvement. This is completely backwards logic.

If you give me the option of a diesel-hybrid high-performance full-sized sports-wagon that gets 120+ mpg (Volvo V60) or a gasoline-hybrid lightweight micro hatchback that gets 50 mpg (Prius) I'll tell you where there is no headroom. Gasoline has hit its development ceiling. And what was point of stating "equal output" right before stating diesel has "higher fuel efficiency figures"? Which is it?

Face it, John, even a Jaguar diesel luxury car driven across America was averaging 60+ mpg and ALL of us know that number would go up significantly if they made it hybrid. That is TRULY exciting ? the opposite of no headroom, that Jaguar could potentially double its mpg. A luxury Jag at 120mpg! Squeezing two or three more mpg out of the miserable Prius is NOT exciting. Diesel engineering is revolutionary and opening up the future of innovation.

The ceiling for gasoline is already here and the improvements are flat and unimpressive. Why would you invest in gasoline hybrid development only to end up with lower mpg than a stock diesel engine? Nonsense. Anyone who has driven a Prius at 65 mph or tried to pull a boat with it knows it never will be as fun or useful to drive as a diesel-hybrid. A Volt could be a completely different car, perhaps even a luxury full-sized car, if it had a diesel instead of gasoline engine.

Last, but not least, John gives us this closing argument.

It's probably significant that Mercedes-Benz, which has sold diesels in the U.S. for many decades, has no plans to sell the world's sole diesel-electric hybrid powertrain here in the States.

If America doesn't get the option then we will never know. The Prius was the sole option and it proved popular. The VW diesel was the sole option and it proved popular. Simple as that; and simple to see why the Volvo V60, which he does not mention, has sold out already for 2013. It's significant that the rest of the world is buying diesel Honda, Acura, Toyota, Lexus, Subaru, Audi, MB?the list goes on and on of options not offered to Americans.

When I drove in 2010 a manual diesel VW Golf in London it felt like I was getting 60+ mpg in a sports car. The inevitable question flashed in my mind: why can't I get this in America? What is really significant is that MB and other manufacturers think Americans are unwilling or unable to recognize diesel as the perfect choice for their profile ? high mileage on open roads with big vehicles hauling stuff. But all it takes is one test-drive and every American I know has fallen in love with new diesel. A diesel hybrid would just make an already awesome option even greater, especially in the city and stop/go traffic.
